We have the tools to get you from A (where you are now) to B (to where you need / want to be) through specialist tailored support. BMS provides Support Coordination, Psychosocial Recovery Coaching, and Therapy Supports to Plan-Managed and Self-Managed NDIS Participants. Support Coordinators empower participants to implement their plans by optimising funding utilisation, navigating service systems, and connecting participants with verified providers tailored to a participant's individual goals.
WHAT TO EXPECT &
BMS OBLIGATIONS
Referral & Ongoing Process
The Participant, their support network, or NDIS submits a referral to BMS. Amanda processes the referral and contacts the participant or support network to schedule the initial meeting.
First Meeting
Plan Review:
Amanda reviews and explains the NDIS plan details to the participant.
Goal Setting:
Amanda collaborates with the participant to identify personal, objectives, daily support requirements, and potential barriers.
Ongoing Action & Coordination
Service Coordination:
Amanda researches information, liaises with community supports, and assists in setting up service providers aligned with the participant's goals.
Reporting & Reviews
Amanda completes the initial plan implementation report at week 8, followed by mid-term reviews and pre-assessment reporting as scheduled.

Psychosocial Recovery Coaches work alongside NDIS participants with a primary psychological disability to navigate complex service systems and build personal capacity. These specialist coaches implement recovery-oriented practice to foster independence, coordinate support networks, and strengthen mental health recovery.

Referral & Ongoing Process
The Participant, their family, support network, or Support Coordinator contacts BMS to submit a referral.
First Meeting
Scheduling:
Amanda contacts the participant to arrange an initial meeting.
Location:
The participant chooses a safe, comfortable setting, such as their home, a local cafe, or Amanda's office space.
Building Rapport
Connection:
This initial phase focusses on establishing trust, open communication, and a strong working relationship between the participant and Amanda.
Needs Assessment
Discussion:
Amanda explores the participant's daily routine, personal strengths, goals, and current psychosocial barriers.
Planning & Action
Coordination:
Amanda helps link the participant to community activities, clinical care services, and other relevant NDIS providers to support their recovery journey.
As a qualified Rehabilitation Counsellor, Amanda combines expertise in person-centered goal setting, deep understanding of functional impacts from mental health conditions, and professional skills in navigating complex support systems.
Rehabilitation Counsellors are trained to look past a clinical diagnosis and focus on how a mental health condition affects a person's day to day functioning, social involvement, and independence. Their training focuses on recovery models, helping participants build motivation, resilience, and self-advocacy oppose to managing symptoms. They excel at working with participants, breaking down life's big obstacles and creating positive steps forward. They know how to connect participants with relevant supports, both community and mainstream supports ensuring a holistic focus.

By focusing on personal recovery goals, this counselling equips participants with practical coping strategies. These services are typically funded under the NDIS Improved Daily Living - Therapy Supports budget category (Rehabilitation Counsellor).
WHAT TO EXPECT &
BMS OBLIGATIONS
Referral, Intake, &Ongoing Process
Contact:
The participant, support coordinator, or healthcare provider contacts BMS to request support.
Suitability Check:
Amanda reviews the request to ensure a rehabilitation counsellor's scope matches the participant's functional goals.
Funding Check:
Verify if the NDIS plan has available funding under appropriate categories (e.g., Improved Daily Living).
Initial Consultation | First Meeting
Location:
Sessions take place at the BMS office, the participant's home, or a mutually agreed-upon safe community space following a risk assessment.
Plan & Funding Verification:
Amanda will confirm how the participant's funding is managed:
Self-Managed - Participant claims reimbursement from the NDIS portal.
Plan Managed - Invoices go directly to the registered plan manager.
Scope of Service:
Rehabilitation Counsellors focus on capacity building, building the participant's own skills, so they can manage complex conditions and circumstances, and function with independence.
Consent & Policies:
Amanda will explain informed consent to the participant and explain the cancellation policy, travel costs and confidentiality. The participant then signs the informed consent form.
Service Agreement & Goal Setting
Service Agreement:
Amanda will present a form to the participant to sign. The form is a formal agreement aligned with NDIS pricing arrangements. It outlines session costs, estimated frequency, and responsibilities.
Baseline Assessment:
Amanda will collaborate with the participant to set measurable, functional, and person-centered goals tied to their NDIS plans.
Ongoing Therapy & Clinical Notes
Capacity Building:
Amanda will deliver evidenced-based interventions focusing on skills development, community access, or vocational goals.
Clinical Records:
Amanda will maintain clear, secure clinical notes after every session to track progress and justify NDIS funding utilisaiton.
Progress Reporting
Review Reports:
Amanda will write concise, objective progress and functional capacity reports 4 - 6 weeks before a participant's NDIS plan review.
Outcome Focus:
Amanda will highlight how therapy has increased the participant's independence and whether ongoing supports are required.
Invoicing
Amanda will provide an invoice the participant's funding allocation:
Self-Managed - invoices are sent directly to the participant for portal claims.
Plan Managed - invoices are sent directly to the plan manager with matching NDIS line items.
